  Title:  GELTH Upstream Observation Well Abandonment  
  Location:  The work to be performed is located at five locations at MCD’s five dams in Ohio:
• Germantown Dam is in Montgomery County, German Township at Conservancy Road north of Creek Road near Germantown, Ohio 45327.
• Englewood Dam is in Montgomery County, Butler Township at U.S. 40 east of S.R. 48 near Vandalia, Ohio 45377 and Englewood, Ohio.
• Lockington Dam is in Shelby County, Washington Township at Kaser Road and Fessler Buxton Road north of Piqua, Ohio 45356.
• Taylorsville Dam is in Montgomery County at U.S. 40 west of Bridgewater Road in Vandalia, Ohio 45377.
• Huffman Dam is in Greene County, Bath Township off of S.R. 4 east of the interchange with S.R. 444 near Dayton, Ohio 45424 and Fairborn, Ohio.

 Description:  The purpose of the GELTH Upstream Observation Well Abandonment project is to abandon twenty-nine (29) observation wells located at the five MCD flood control dams. The wells range in diameter from 2 through 7 inches. The wells range in depth from approximately 12 through 139 feet. The work to be done under this project includes:
A. Prepare appropriate cement grout.
B. Use tremie pipe to grout well shut from the bottom up.
C. Cut well pipe off 3 feet below ground surface and dispose of pipe.
D. Demolish any concrete pad around observation well(s).
E. Finish area to grade as appropriate.
F. One of the 29 wells to be abandoned will require that the pipe be removed from the ground.
 
 Requirements:  Bids must be submitted on MCD Proposal Forms (pages 15–23 printed single-sided) and must be accompanied by a bid guaranty and Ohio Department of Insurance Certificate of Compliance. Do not submit the entire Contract Documents/Specs.

The chosen lowest and best bid Contractor will provide performance/contract bond, insurance, and Workers' Comp before Contract is awarded.

The Contractor and all Subcontractors will abide by the Contract Documents and all local, state, and federal laws pertaining to this project.

LEGAL ADVERTISEMENT & NOTICE TO BIDDERS

Sealed proposals will be received at the office of The Miami Conservancy District (MCD) 38 East Monument Avenue, Dayton, Ohio 45402-1265 until 2:00 PM local time, Friday, December 18, 2015, for Contract No. 2015-044C-400.000, GELTH Upstream Observation Well Abandonment project located at MCD’s five dams: Germantown Dam, Montgomery County near Germantown, Ohio; Englewood Dam, Montgomery County near Englewood, Ohio; Lockington Dam, Shelby County north of Piqua, Ohio; Taylorsville Dam, Montgomery County in Vandalia, Ohio; and Huffman Dam, Greene County near Fairborn, Ohio.

Bids must be submitted on MCD Proposal Forms and must be accompanied by a bid guaranty to the benefit of The Miami Conservancy District. The bidder has an option of the bid guaranty. The bid guaranty may be a bid bond in an amount equal to one hundred percent (100%) of the amount of the bid. The bid bond shall be secured by a guaranty or a surety company licensed as surety by the Ohio Department of Insurance. The bid guaranty may also be a certified check, cashier’s check, or letter of credit in an amount equal to ten percent (10%) of the amount of the bid drawn to the Treasurer of The Miami Conservancy District. No bid will be considered unless it is accompanied by the required guaranty as a guarantee that the bidder after such acceptance will furnish an approved surety bond for the faithful performance of the Contract. The lowest and best bid will be accepted, except that the right is reserved to reject any or all bids and to waive any technical defects as the interests of MCD may require.

Plans, Specifications, and Proposal Forms may be examined and obtained on MCD’s website at www.mcdwater.org under Construction Projects or examined in the office of the Chief Engineer of The Miami Conservancy District, Monday through Friday between the hours of 8:30 AM and 4:30 PM.  Contract Documents/Specs, and Proposal Forms may be printed free of charge from MCD website upon becoming a member of this project. There are no MCD Drawings/Plans referenced with this project.
